Influence a vacuum-assisted closing of defects of malignant tumors of skin after an epidermization on local recurrence-free survivability

Summary. Free skin shreds are one of the best variants for the reconstruction of large defects of skin after removing of malignant tumors from the achievement of clean edges of resection. The no less survival of this type of the plastic arts often becomes a call in practice of surgeon. During great while accessible for the improvement of results of cicatrization of postoperative wounds is a closing method with the use of vacuum (vacuum-assisted closure — VAC) that provides far the best survival of free skin shred. The aim of our research was to estimate influence of the vacuum-assisted systems on the origin of local relapses by weighing of local survival and taking into account of co-factors of influence on given. Materials and methods. In basis of research the analysis of results of treatment is fixed a 81 patients with malignant new formations of skin, that were on treatment in Nonprofit Organization National Cancer Institute of 2019–2023 patients is up-diffused on 2 groups depending on the use of VAC dressings for the improvement of survival of free skin shred: group 1st — with raising of VAC dressings, group 2nd — without raising of VAC dressings. Results. On results a supervision in a postoperative period the comparative analysis of frequency of origin of local relapses and estimation of local survival is executed between groups. Survival of patients of group 1 laid down 42/47 (89/4 and patients 2 groups (with VAC) — 30/34 (88/2) by the unmeaningful statistical difference (of р=0/896, Logrank of test). Relation of risks (RR) (of 1/2)=1.09 (95% confidence interval (CI) 0.29–4.1), probability of survival without progress in a group 1 on 9% more in comparing to the group 2nd. Conclusions. Use of VAC dressings after transplantation of free skin shred in practice surgical treatment of malignant tumors of skin does not worsen the indexes of oncologic control. The compared tendency in relation to graft survival testifies in behalf on the use of the offered method.
