Peculiarities of treatment of patients with laryngeal cancer

Summary. The incidence of laryngeal cancer in Ukraine was 4.4 per 100,000 population in 2021. Most often, laryngeal cancer is found in men of working age. In (59.5–63.2)%, stage III–IV laryngeal cancer is diagnosed. This affects the choice of treatment method for such patients. Goal. Determination of the effectiveness of surgical and conservative treatment of patients with laryngeal cancer. Object and methods. 253 patients with laryngeal cancer were examined. 127 patients had stage I–II laryngeal cancer. 126 patients had stage III–IV cancer of the larynx. In all cases of cancer, the diagnosis was confirmed histologically. The average age of the patients was 59.5 years. All patients were divided into two groups: surgical and conservative treatment. The result of treatment of patients with laryngeal cancer was evaluated. Dynamic observation was three years. The results. Metastasis to lymph nodes of the neck was significantly more characteristic of cancer of the supraglottic part of the larynx (χ2=55.859; p <0.05). Event-free survival of laryngeal cancer patients was 60% in the surgical group and 44% in the conservative treatment group (χ2=8.104; p <0.05). The median event-free survival was only in the group with conservative treatment of patients and was 21 months. The overall survival rate of patients in the group of surgical treatment was 90%. In the group of conservative treatment — 78.2%. However, there was no statistical difference (χ2=2.846; р >0.05). Conclusions. Metastases to regional lymph nodes are more often observed in cancer of the supraglottic part of the larynx. Event-free survival of patients with laryngeal cancer was significantly higher after surgical treatment of patients.
