Summary. Breast cancer (BC) is one of the leading causes of cancer-related morbidity in female worldwide. Materials and methods. 95 patients with estrogen positive BC were included in the study, 46 patients were HER2/neu negative and 49 — HER2/neu low. All patients received neoadjuvant chemotherapy with AC-T regimen. Tumor infiltrating lymphocytes (TIL) were assessed in core biopsy and surgical specimens, stromal and intratumoral CD4+, CD8+ were evaluated. Results. Tumors in patients with HER2 low BC had higher level of TILs compared to HER2 negative tumors (р=0.002). In patients with HER2 low BC after neoadjuvant chemotherapy was observed statistically significant increase of stromal CD8+ lymphocytes (р=0.01) and decrease of intratumoral CD4+ lymphocytes. These findings indicate the possibility of chemotherapy to increase HER2 low BC immunogenicity. Conclusion. The results of the study show the preliminary features of immune microenvironment in Her2 low BC and require further evaluation.
