Evaluation of the quality of life of patients after surgical interventions for complicated forms of colon cancerKubrak M.A., Zavhorodniy S.M., Danylyuk M.B. Summary. The aim of the study. To assess the quality of life of patients who were operated on for complicated forms of colon cancer in a general surgical hospital. Object and methods. The study group included 71 (100%) patients who were operated on for complicated forms of colon cancer. In 49 (69.1%) patients, a colon tumor with acute intestinal obstruction was detected, in 13 (18.3%) — with perforation of the tumor, in 3 (4.2%) — with acute intestinal bleeding, in 4 (5.6%) — a combination of acute intestinal obstruction with tumor perforation, another 2 (2.8%) — intestinal obstruction with bleeding. A colostomy was performed in 42 (59.2%) patients. The quality of life of patients evaluated by using the «SF-36 Health Status Survey» questionnaire in 6 months after the surgical intervention. Research results. According to the results of the questionnaire of 59 (83.1%) patients, the average level of the physical component of health in the studied group was 43.6 (40.7; 47.1) points, the indicator of the mental component of health was at the level of 45.8 (43.1; 49.6) points. The indicator of the physical component of health among patients who were operated on with an ostomy was at the level of 42.8 (39.9; 45.3), without an ostomy — 44.1 (41.3; 48.9) points, U=185.0; p=0.2738. The indicator of the mental component of health — 40.9 (37.1; 42.8) points in patients with an ostomy and 47.3 (43.5; 49.4) points in patients without an ostomy, U=415.0; p=0.0081. Conclusions. 1. The average level of the physical component of health 6 months after the intervention for complicated forms of colon cancer in the study group was 43.6 (40.7; 47.1) points, the indicator of the mental component of health was at the level of 45.8 (43.1; 49.6) points. 2. The lowest indicators of the quality of life according to the SF-36 scale were noted in the mental component of health, namely in life activity (VT), social functioning (SF) and role functioning determined by the emotional sphere (RE). In the physical component of health, the lowest indicators were noted in role functioning due to the physical sphere (RF). 3. Surgical intervention for complicated forms of colon cancer, which ends with the imposition of an ostomy, significantly worsens the quality of life of patients without an ostomy due to the mental component of health (р=0.0081), while not affecting its physical component (р=0.2738). 4. Deterioration of the mental component of health in patients with a stoma occurs due to a decrease in vital activity (p=0.0437), social functioning (p=0.0362) and role functioning due to the emotional sphere (p=0.0178).
